Was your adoption in Florida or Alabama? Do you know if your birth-mother was in a maternity home?
Here is some information from another site:
How to obtain Original Birth Certificate in
Alabama:
If your adoption was finalized in the State of Alabama, you are entitled to receive a copy of your OBC (Original Birth Certificate) along with pertinent court documents that were filed at the time of your adoption.
Although there IS certain information that is available to you from the Department of Human Resources, Post Adoption Office -- such as non-ID, and other inquiries -- this office is NOT the one to contact to request your OBC.
To accomplish this click to the web site for Alabama's Center for Health Statistics: http://ph.state.al.us/chs/VitalReco...ion/adopt.html; scroll to the bottom of the page; click on the box entitled "Adoptee Seeking Original Birth Information."
At the above link, you can print out the required form; this form can only be submitted to the Center for Health Statistics via mail --no "walk in" requests are honored. The only requirement is that the adoptee be 19 years of age or older, and the request form must be accompanied with a non-refundable fee of $20. You do not have to know your birthmother/birthparents' names ... requirements from the above website:
-Your full name as it appears on your current legal birth certificate
-Date of birth
-County of birth
-Full maiden name of mother as it appears on your current legal birth certificate
-Full name of father as it appears on your current legal birth certificate
At the above website do NOT click on the link to order your Birth Certificate via the web. If you do, you will receive a copy of your Amended Birth Certificate ... the one that was issued after your adoption was finalized.
